Insurance
- Check with your insurance company. Confirm that your plan is accepted at our practice and that you understand your coverage, copays, and deductible.
- HMO patients: If you do not have a referral, please change your Primary Care Physician (PCP) to Dr. Hussein Anan (One Hand 360) before your visit. Without a referral or this PCP change, you may have to pay out of pocket.
Complete Pre-Check-In Online
- Pre-check in through the Privia website before your appointment. This saves time at the front desk and helps us get you in to see the doctor sooner.
- Fill in and sign all consent forms ahead of time.
What to Bring
- Photo ID (driver’s license or other government-issued ID)
- Insurance card(s), including any secondary insurance
- A current list of all your medications, including dosages, over-the-counter drugs, vitamins, and supplements
- Referral paperwork, if your plan requires one
- Any relevant medical records, imaging, or test results from other providers
- A form of payment for your copay or any balance due
Helpful Tips
- Arrive 10-15 minutes early, especially if it is your first visit.
- Write down your questions and symptoms so nothing gets forgotten.
- Let us know of any changes in your insurance, address, or phone number.
- If you need to cancel or reschedule, please call us as early as possible so we can offer your time to another patient.
